the real consolidated documentation.
Delete old Doxygen-based rpkid manual and stuff that went with it,
while retaining ability to generate Doxygen manual of internals for
the libraries.
This closes #232.

invalid XHTML, but the specific errors xsltproc is complaining about
at the moment don't affect lynx's text dump. Sigh. Least dangerous
option is to filter xsltproc's stderr to suppress (just) the known
harmless error messages.
